Permit to work forms are an integral part of a comprehensive Health and Safety Management system, each permit form records hazards and precautions relevant to the specific job. These forms contain three parts, the original form plus two carbon copies as well as a card backing insert for separating the carbon copies. Supplied in packs of 10.

1. Type of Permit (Location, Date Issued, Permit Number, Plant Identification, Start time and Finish time).
2. Safety Precautions (Precautions, Protective Equipment, Services to be isolated, COSHH and Lone Working, Daily Inspections and Fire Watch).
